OPEN ENROLLMENT FOR BENEFIT YEAR 2022

The annual open enrollment period for the City of Stow Group Health Benefit Plan is in effect from November 21, 2021, to December 7, 2021.  During this period, eligible employees may enroll or change benefit elections, including adding or removing eligible family members to the City of Stow Group Health Benefit Plan which includes medical and dental coverage.


EMPLOYEE MONTHLY PREMIUM CONTRIBUTION RATES FOR 2023:

Single health care coverage for medical, dental or both | $127.50 per month ($63.75 per pay – first two pays a month)

Family
health care coverage for medical, dental or both – $255.00 per month ($127.50 per pay – first two pays a month)

An additional spousal surcharge will be deducted if the following applies: (no change from 2022)

·        Stow employees hired before January 1, 2014,
will be subject to a monthly spousal surcharge in the amount of $50.00 per month.

 

·        Stow employees hired on or after January 1, 2014,
will be subject to a monthly spousal surcharge in the amount of $100.00 per month.

 
Spousal surcharge notices must be completed by all employees electing family coverage. You may download the form from the resource tab or a form will be sent to you by the City.

Employee deductibles/coinsurance amounts for 2023
have been established and are as follows:

Single Deductible $450
Coinsurance Percentage 20% up to $350
Maximum Annual out-of-pocket $ 800

Single additional deductible of $500.00 if utilizing out-of-network providers


Family
Deductible $900
Coinsurance Percentage 20% up to $700
Maximum Annual Out-of-Pocket $1,600

Family additional deductible of $1,000if utilizing out-of-network providers.

 

As in prior years, if a qualifying event (birth, death, marriage, adoption, divorce, termination, etc.) occurs which may affect coverage of the employee or family member, employees are required to notify the City of Stow of such an event as soon as possible but not longer than 30 days from the date of the event. This allows the City to may make the necessary changes to the health care plan and applicable employee deductions.  A qualifying event may fall outside of the enrollment period, but still needs to be updated with the City.

New for 2023! 
The City of Stow will be using Medical Mutual of Ohio for the Flexible Spending Program.  FSA Elections must be updated on an annual basis, you will need to complete the election form under the FSA information to elect this benefit
for 2023.
